Inclusion Criteria
- •Aged 18 years old and over
- •Have a known history of diabetes
- •Reside in the CESPHN region
- •Belong to the target priority population but not limited to (People experiencing homelessness, Aboriginal and Torres Strait Islander peoples and culturally and linguistically diverse people)
- •\- all staff involved in the project
Exclusion Criteria
- •Aged \<18 years old
- •Do not reside within the CESPHN region
