nursing home residents at least 65 years old or older, written informed consent, prescribed pharmacotherapy

Exclusion Criteria

less then 65 years old, no informed consent, acute life threatening situation or isolation due to multidrug resistant infectious disease

Primary outcome: Appropriateness of medication measured by MAI (medication appropriateness index) at T0 (before Intervention), T1 (after 12 months),T2 (after 18 months)

Secondary endpoints are:<br>1) The registration of occurence of delirium measured with the Delirium Observation Screening Scale (DOS, Schuurmans et al. 2003) und the Confusion Assessment Method ( CAM; Inouye et al. 1990; german Translation: DOS, CAM Hasemann et al. 2007)<br>2.)The resident`s mobility, registered with the timed up and go test (TUG, Podsiadlo et al. 1991) and tendency of falls<br>3.) severe medication interactions registered with suitable medication interaction databases<br>4.) potential malnutrition registered with the Simplified Nutritional Appetite Questionnaire (SNAQ, Wilson et al. 2005) and possible unintentional weith loss of residents within the regular documentary system oft he nursing homes<br>5.) evaluation of the resident`s health related life quality (measured with EQ-5D).<br>6.) The (expected) improvement of interprofessional cooperation and communication will be evaluated with qualitative guided group interviews.<br><br>
