简要总结
Parkinson’s disease (PD) is the second most common age- related neurodegenerative disease, it is characterized by Tremors, Rigidity, Bradykinesia and Gait dysfunction with Postural instability, Speech difficulty, swallowing impairment, and series of non-motor features that include Autonomic disturbances, Sensory alterations, Mood disorders, Sleep dysfunction, Cognitive impairment, and Dementia. Parkinson’s disease can be taken parallel to Kampavata mentioned in Ayurvedic classics, where the
lakshanas like Karapadatala Kampa, Dehabhramana Dukhitha, Nidrabhanga, Matiksheena are quoted. The incidence and prevalence of PD are rising. In the year 2021, the estimated global prevalence of PD was11.8 million people, increased from 6.1 million in 2016 and projected to reach 25.2 million by 2025. A similar trend has been observed in age-adjusted mortality from PD. Although aging of the world population accounts for much of the increase in absolute numbers, the age- adjusted incidence is also rising, for reasons that are not fully understood. The mean onset of Parkinson’s disease is at the age of 60 years, and lifetime risk is 3% for man and 2% for women. Treatment for PD is dopamine replacement therapy – Levodopa or Carbidopa, Other drugs like Amantadine, Monoamine Oxidase type B Inhibitors (MAO-B), Anticholinergic drug, Dopamine Agonists, COMT Inhibitors are used as the disease progresses. These medications may have complications on long term usage. Here the need of Ayurveda for better management of the condition and improving the quality of life. Acharya Vangasena stated the principles of the treatment for Kampavata as Abhyanga, Swedana, Nasya, Niruha, Anuvasana, Virechana and Shirobasti which is the main treatment for vata dosha and even increases the quality-of-life expectancy of patients suffering from Kampavata. In Kampavata increased Chala Guna of Vata produces Kampa whereas increased Manda Guna of Kapha produces Chestasanga.
In Ayurvedic Classics, treatment protocol for Kampavata can be considered as Samanya Vata Vyadhi Chikitsa. Here Snehana Karma is adopted as Pana and Nasya in the form of Taila, which is having the properties of Vyavayi, Ushna, Guru and Snigdha Guna which is best for all Vata Vyadhi. Mahakalyanaka taila is specifically indicated in conditions such as Vepathu, Ardita, Dhanusthambha, Kubjata. It contains ingredients like Bhallataka (Semecarpus anacardium), Ashwagandha (Withania somnifera), which possess CNS Stimulant and Neuroprotective properties.Hence in this study, an attempt is made to study the combined effect of Mahakalyanaka Taila as Nasya and Pana in PD and in improving the quality of life.
