1. All children admitted to the four participating hospital's inpatient units general hospital wards (excluding high dependency and intensive care units)
2. The parents of some of these children
3. The healthcare professionals looking after some of these children
1. Preterm infants (<37 weeks gestation)
2. Adult patients >18 years of age
3. Patients not admitted to the hospital (e.g., those only seen in AED and discharged)
4. Patients admitted directly to PICU or HDU
5. Temporary or agency staff working in these departments

Name Time Method <br> The primary outcome measure is the monthly collected rate of mortality and the following critical events:<br> 1. Unplanned admission to Paediatric Intensive Care (PICU) or Paediatric High Dependency Unit (PHDU)<br> 2. Cardiac arrest<br> 3. Respiratory arrest<br> 4. Medical emergencies requiring immediate assistance (arrest calls who were not respiratory or cardiac arrests)<br> 5. Referrals for PICU review (in tertiary centres) or PICU retrieval (DGHs)<br> 6. The Critical Deterioration (CD) metric<br>
