- •Admitted to RGH; identified as requiring Aged Care Assessment Team (ACAT) assessment (or reassessment).
Exclusion Criteria
- •Only the carer will be interviewed if the patient has speech, language \&/or other communication difficulties or severe cognitive impairment.
