primiparous women; age between 18-35 years; singletone and term pregnancy; normal fetus; beginning of the active phase of labor(dilatation of 3 to 4 cm); lack of used narcotic within 3h before and during the intervention; lack of received oxytocin during the intervention; lack of psychological disorders and chronic disease; dermatologic disorders in sacrum; cold allergy; history of any chronic pelvic pain; lack of abnormal pattern of fetal heart rate and obstetric complications (advanced diabetic, hypertension disorders).
exclusioncriteria: occurrence fetal distress or disorder of labor develop during the study.

Name Time Method Pain severity. Timepoint: Before intervention, then every 1 hours after intervention in active phase. Method of measurement: Numeric Rating Scale.;Anxiety. Timepoint: Before intervention, then in 4-8 cm and 8-10cm cervix of dilatation. Method of measurement: Anxiety Inventory State questionnaire.;Birth satisfaction. Timepoint: 24 hours after childbirth. Method of measurement: Birth Satisfaction Scale-Revised Hollins Martin.
- Secondary Outcome Measures
Name Time Method Duration of active phase. Timepoint: Once and from 4cm dilatation to full dilatation and effacement cervix. Method of measurement: Per minute and using standard digital watch.;Duration of second stage of labor. Timepoint: Once and from full dilatation to child birth. Method of measurement: Per minute and using standard digital watch.;Fetal Heart Rate. Timepoint: Every 30 minute in active phase and every 15 minute in second stage. Method of measurement: Using standard sonokids and watch.;Neonatal Apgar score. Timepoint: First and fifth minutes after delivery. Method of measurement: Observation and examination, using Apgar score table.;Neonatal weight. Timepoint: immediately after delivery. Method of measurement: Per gram and using standard balance.
