- •1\. Synchronous or metachronous (within 5 years) malignancies other than carcinoma in situ or mucosal carcinoma 2\. Cases with uncontrollable diabetes 3\. Cases with uncontrollable congestive heart failure, angina pectoris, hypertension, arrhythmia 4\.
