Accepted into Heart Failure Service at Princess Alexandra Hospital
Exclusion Criteria
Unable to read and write English
Unable to view DVD
Significant cognitive impairment
Living in a nursing home or dependent housing
Being considered for transplantation or artifical heart device
Terminal malignancy
