To compare the effects of volume-controlled ventilation with I:E ratios of 1:1.5, 1:2, and 1:2.5 on respiratory mechanics in patients undergoing laparoscopic surgeries.

Summary This prospective, randomized clinical trial compares the effects of three different inspiratory-to-expiratory  ratios , 1:1.5, 1:2, and 1:2.5 , during volume-controlled mechanical ventilation in adult patients undergoing laparoscopic surgeries.

A total of 75 patients , ASA I and II, aged 18 to 59 years, will be randomly assigned to one of the three groups. Standard anesthesia will be administered, and ventilation settings will be adjusted according to group allocation. Primary outcomes include respiratory mechanics such as peak airway pressure, plateau pressure, dynamic compliance, and airway resistance. Secondary outcomes include gas exchange parameters and hemodynamic stability during pneumoperitoneum.

The study aims to identify the optimal I:E ratio for improving lung mechanics and oxygenation while maintaining cardiovascular stability during laparoscopic upper abdominal surgeries. The findings are expected to guide anesthesiologists in selecting evidence-based ventilation strategies that may reduce postoperative pulmonary complications.

入选标准

  • ASA physical status I and II 2) Patients scheduled for elective laparoscopic surgeries 3) BMI 18.5 to 30 kg per metre square.

排除标准

  • Patient refusal 2) History of COPD, asthma, or restrictive lung disease 3) History of significant cardiovascular disease 4) Severe hepatic or renal impairment 5) Pregnancy 6) Emergency surgeries • BMI greater than 30 kg per metre square.

To compare the effects of volume-controlled ventilation with I:E ratios of 1:1.5, 1:2, and 1:2.5 on respiratory mechanics in patients undergoing laparoscopic surgeries.

时间窗: 0 min, 15 mins, 30 mins, 60 mins, after desufflation

  • To evaluate the impact of these three I:E ratios on gas exchange parameters and hemodynamic variables during pneumoperitoneum.(0 min, 15 mins, 30 mins, 60 mins, after desufflation)
