Patients who had the following criteria are excluded: (1) polymicrobial bacteremia; (2) immediately transferred to other hospital; (3) no data on treatment outcome; (4) died within 72 hours after index culture; (5) received renal replacement therapy; (6) received vancomycin for less than 72 hours; (7) not receiving vancomycin within 5 days after index culture; (8) vancomycin serum concentration was not checked within the first 5 days of therapy; (9) unavailable index MRSA isolate.

- Primary Outcome Measures
Name Time Method Vancomycin treatment failure was defined as one of the following criteria : (1) 30-day all-cause mortality, (2) persistent bacteremia (>=7 days of vancomycin) (3) recurrence of bacteremia (<=30 days);initial steady-state vancomycin AUC24/MIC
- Secondary Outcome Measures
Name Time Method vancomycin MIC;vancomycin trough level
