1. Patients are equal or older than 60 years old.
2. Vascular patient that have plan to operate (aorta, carotid and peripheral) who get anesthesia more than 2 hours.
3. Elective case
1. Patients have cognitive impairment before surgery who get less than 24 score of Thai Mental State Examination (TMSE)
2. Patients who regularly drink alcoholic beverages and drink within the 14 days before surgery.
3. Pre-operative patients who have positive delirium when test with Confusion Assessment Method for Intensive Care Unit (CAM-ICU)
4. Patients with creatinine clearance < 30 ml/min
5. Patients with hepatic impairment with Child Pugh score greater than 10
6. Patients with history of risperidone allergy or history of neuroleptic malignant syndrome
7. Emergency case
