Eligibility Criteria

Inclusion Criteria

  • •1\. Inpatient volunteer with chronic illness, age 10\-19 years old
  • •2\. Plan to take two or more oral medications for at least 4 weeks
  • •3\. Clinically stable
  • •4\. Able to communicate Thai

Exclusion Criteria

  • •1\. Already had medication chart
