- •(2) Neck or thoracic region pain within the last year.
- •(3) Headaches within the last year that resulted in limitation of daily activities.
- •(4) History of spinal surgery to the thoracic or cervical spine.
- •(5)Temporomandibular joint (TMJ) dysfunction.
