Inclusion criteria for geriatric patients:
• Patients =70 Years with at least two geriatric feature complexes (i.e. = 2 points in the ANGELINA questionnaire from at least two different topic complexes)
• sufficient German language skills
• For patients who are being cared for in health care and/or have a consent requirement, the caregiver must also sign the consent form.
Inclusion criteria for practices:
• The doctor must be a member of the respective practice network.
• be cash approved
• Participants can be general practitioners of internal medicine, general practitioners and general practitioners.
Exclusion criteria for patients at the time of recruitment:
• Patients living in inpatient care facilities
• Patients with a terminal disease in the last phase of life and/or use of specialized palliative care (SAPV, hospice)
• Patients with a bipolar disorder or other serious psychiatric illness.
Exclusion criteria for the practices:
• Exclusively private medical practices

Name Time Method Measuring the ability to cope with everyday activities with the Barthel Index (assessment instrument)

Name Time Method 1.) Determination at patient level:<br>- Coping with daily household tasks measured with the IADL questionnaire<br>- Mobility measured with Timed up and go<br>- Falls detected with a fall log<br>- Quality of life measured with WHOQOL-OLD/ and -BREF<br>- Nutritional status measured with the MNA-Elderly<br>- Determine treatment satisfaction of the geriatric patients in the intervention group as well as utilization of care services and associated costs.<br>2.) Determination at the level of affiliates and associates:<br>- Load measured with the Domestic care scale.<br>- Determine the level of support<br>- Record treatment satisfaction<br>3.) Determination at the level of the service provider:<br>- Measuring the duration between the identification and implementation of a care need<br>- Reduction of interface problems (cross-sectoral) between different service providers<br>- Query satisfaction with the supply situation<br>- Derive improved teamwork through delegation
