Eligibility Criteria
Inclusion Criteria
- •Patients aged 18 years and older, using 5 or more chronic medicines, discharged from the hospital (departments of surgery, internal medicine, cardiology), with 1 or more changes in their chronic medication
Exclusion Criteria
- •Patients discharged to a nursing home, patients with a life expectancy shorter than 6 months, patients with a language barriere, patients not willing or unable due physical/mental constraints to participate in this study, patients' community not enrolled in the study
