入选标准
- •1. Patients suffering from two or more of the chronic conditions
- •2. Patients who are under usual care for the suffered chronic conditions.
- •3. Ability to comply with the study protocol
- •4. Voluntary written Informed consent
排除标准
- •1. Patients suffering from only one of the clinical conditions.
- •2. Inability to comply with the study protocol.
- •3. Severe mental health problems: schizophrenia, bipolar affective disorder, or other psychotic illnesses.
- •4. Life-threatening illnesses, malignant tumours, HIV etc.
- •5. Patients, at the time of enrolment, should not be suffering from any acute clinical condition that has no association with their chronic ailment
