1. Patients suffering from two or more of the chronic conditions
2. Patients who are under usual care for the suffered chronic conditions.
3. Ability to comply with the study protocol
4. Voluntary written Informed consent
1. Patients suffering from only one of the clinical conditions.
2. Inability to comply with the study protocol.
3. Severe mental health problems: schizophrenia, bipolar affective disorder, or other psychotic illnesses.
4. Life-threatening illnesses, malignant tumours, HIV etc.
5. Patients, at the time of enrolment, should not be suffering from any acute clinical condition that has no association with their chronic ailment

Name Time Method The primary outcome will be the health-related quality of life (HRQoL) as measured by the EQ-5D-5L utility scoreTimepoint: Baseline, Month 12
- Secondary Outcome Measures
Name Time Method To evaluate changes in HRQOL using EQ-5D-5L, PROMIS 29, BIPQ, MULTIPLEs questionnaires at different time points. Incidences of hospital admissions among the two groups. Reduction of usual care drugs, differences in the cost of medical expenditure, changes in laboratory parameters among the two groups. Occurrence of adverse events.Timepoint: Baseline, and over 12 months
