Eligibility Criteria

Inclusion Criteria

  • •Patient/Attendant Giving Consent
  • •\-All Adult Patient Admitted \>72 Hours In ICU , Either Directly Discharged to home or transferred to ward.

Exclusion Criteria

  • •\- Post Operative patient With surgery \<1 week
  • •\- Patient Discharged from ward within 24 hours of transfer
