Inclusion Criteria
- •Moderate\-severe COPD
- •\>\=1 hospital presentation in previous 12 months
- •English language fluency
- •Willing to use a computer for health self management
- •Living independently
Exclusion Criteria
- •Significant co\-morbidities including cancer, renal failure, cognitive impairment.
