1Diagnosed with type 2 DM by physician at the medical record
2Age between 30-59 years old
3Having HbA1C above 7%
4Having the atherosclerotic cardiovascular disease (ASCVD) risk score equal or above 7.5%
5Be able to perform activities of daily living by oneself
6 Be able to communicate in Thai language
7Be able to contact via telephone
8 Committed significant family member involved into the program
9 Be willing to participate and give informed consent in the study
1.Having a severe complications and other serious condition which made them unable to participate the program such as hospitalization, heart disease, volume overload, DKA etc.
2.Having insulin injection medication
3.Refusing to participate
