Eligibility Criteria
Inclusion Criteria
- •1\. Patients receiving home medical care
- •2\. Patients who fall under the surprise question by the doctor in charge
- •3\. Patients and family members must be at least 20 years of age at the time of consent
- •4\. The research subject or his / her family owns an electronic medium on which ePRO type ACP can be implemented.
- •5\. Patients who have given written consent to participate in this study
- •Patients who meet the above five items, their families, and intervening healthcare professionals are the subjects of this study.
Exclusion Criteria
- •1\. Patients who are unable to create ACP due to impaired consciousness or dementia
- •2\. The official ACP has already been completed
- •3\. Patients who are judged to be ineligible at the discretion of the researcher
- •Exclude patients who fall under any of the above three items, their families, and intervening healthcare professionals
