Eligibility Criteria
Inclusion Criteria
- •1\) Patients with a (ischemic or hemorrhagic) stroke that have to make a decision regarding discharge location and type of care after discharge from the hospital; 2\) Age \= 18 years; 3\) Able to understand the Dutch language in speech and writing.
Exclusion Criteria
- •1\) Dementia (based on clinical assessment); 2\) Reduced consciousness (based on clinical assessment).
