Primary Outcome Measures

Record how patients with a newly diagnosed dementia after a hospital stay are cared for in the GP setting and how this care can be improved if necessary. Secondary Outcome Measures
NameTimeMethod
Two focus group discussions (one discussion with GPs and one discussion with patients and relatives) with the aim of expanding the empirical data corpus from the interviews, as well as the triangulation of the differing data and subsequent qualitative content analysis (January to March 2022 = 3 months). In addition, an expert workshop serves to validate the practice-oriented recommendations for the GP setting (July 2022 = 1 month).
