Inclusion Criteria
1) Patients above 18 years admitted in medical IC
Exclusion Criteria
1) Age less than 18 years
2) Known severe carotid stenosis ( >50%) or
occluded carotid arteries
3) Inadequate transcranial doppler window
4) Pregnant patients
5) Patients with history of Mental retardation
6) Patients not expected to survive for > 24
hours in ICU
7) Cardiac arrythmias at the time of measurement
8) Penetrating trauma to the head or significant
ocular trauma
9) Refusal of consent

- Primary Outcome Measures
Name Time Method Describe the Pulsatility index (PI), Resistivity index (RI), MCA - Peak systolic velocity (Psv), End diastolic velocity (Edv), Mean flow velocity (Mfv), ICP and Cerebral Blood flow index (CBFi) as measured by TCD among patients being admitted in ICUTimepoint: Patients shall be assessed <br/ ><br>within 24 hours post admission and subsequently every 24 hours till discharge from ICU or death.
- Secondary Outcome Measures
Name Time Method a)To determine if TCD parameters -PI, RI, Mfv, Psv, Edv, ICP (PI derived) and CBfi (PI derived would improve the predictive ability of severity of illness score such as APACHE II. <br/ ><br>b)To determine whether the pattern and the distribution of these parameters will help differentiate the pathophysiology of encephalopathy in these patients <br/ ><br>Timepoint: Patients shall be assessed within 24 hours post admission and subsequently every 24 hours till discharge from ICU or death.
