- •2\.Having physician diagnosis of or on treatment for cardiovascular disease including ischemic heart disease, ischemic stroke, or heart failure regardless of ejection fraction
- •3\.Living within same city/town
- •4\.Consent to participate.
排除标准
- •1\.Pregnant women
- •2\.Patients having diseases associated with frequent hospitalization (advanced cancer, end stage renal disease); bedridden or debilitating conditions and life expectancy \<12 months that might limit completion of study follow\-up
