Inclusion Criteria

Age > 18 years.

Intubated.

Receiving Mechanical Ventilation in ICU > 48 hours.

COPD Patients of type 2 respiratory failure.

Patients with Maximum inspiratory pressure (PIMAX): 15 to 30 cm H2O.

Rapid shallow breathing index (RSBI): 60â??105

Exclusion Criteria

Unable to perform test manoeuvre.

Any pre-existing neuromuscular disease.

Any Associated Pulmonary Disease.

Acute coronary syndrome (ACS).

Haemoptysis.

Spinal cord injury above T8.

Any skeletal pathology that impairs chest wall movements such as severe kyphoscoliosis congenital deformities or contractures.

Poor general prognosis or anticipated fatal outcome.
