Inclusion Criteria

All children from 1 months to 59 month with severe pneumonia( modified WHO guildelines 2014) and hypoxemia and present in paediatric ICU;

1.Cough with/without fever with fast breathing and/or chest indrawing AND

2.Any 1 of the General Danger signs (lethargy, reduced level of consciousness , convulsion, not able to drink, persistent vomiting)

AND

3.Hypoxaemia (SPO2 <90% at room air) at time of enrolment.

Exclusion Criteria

1.Children requiring Mechanical Ventilation or intubation at presentation

2.Children already exposed to HFNC or CPAP before coming to PICU

3.Children with craniofacial abnormality or maxillofacial surgery (cleft palate, choanal atresia etc)

4.Children with air leak syndrome (pneumothorax, pneumomediastinum, subcutaneous emphysema)

5.Children with neuromuscular diseases

6.Known case of asthma or upper airway obstruction

7.Children with tracheostomy

Proportion of children developing Respiratory support Failure in CPAP and HFNC in first 72 hours.Timepoint: Monitoring would be done at baseline, hourly for the first 6 hours and then at 12, 18, 24, 36, 48 and 72 hr of initiating respiratory support
