- •1.Type 2 DM patients have FBS > 150mg/dl or HbA1C > 7%
- •2.Functionally independent patient, Barthel index > 12
- •3.age 35-80 years
排除标准
- •1.Functionally dependent patients
- •2.Neurologically conditions eg. pakinson disease, dementia, stroke, hemiplegia
- •3. Other immunocompromised host or use immunosuppresive drugs or steriod
- •4.cannot contact with telephone or line.
