•ASA I- III Patients undergoing Endoscopic Retrograde Cholangiopancreatography (ERCP) procedures for biliary or pancreatic interventions Weight > 30 kg Anticipated procedure time > 30 min Fasting at least 6 hrs for solids and 2 hrs for clear liquids

•Refusal of consent Restricted mouth opening (inter-incisor distance < 3 cm) where LMA cannot be inserted Pregnancy BMI > 35 kg/m2 Altered consciousness (GCS < 8) Left ventricular ejection fraction less than 50 % Patients at significant risk of pulmonary aspiration (i) increased gastric content as a result of delayed gastric emptying including in situ lap band, gastric hypersecretion and emergency surgery; (ii) increased tendency to regurgitate because of symptomatic or uncontrolled gastrooesophageal reflux, oesophageal strictures, Zenker’s diverticulum, or achalasia; and (iii) laryngeal incompetence because of head injuries, cerebral infarct, cerebral palsy, Parkinson’s disease, and other neuromuscular disorder Procedure abandoned or not done after induction of GA within 30 mins
