Inclusion Criteria
- Thai adults aged 18-60 years old
- Diagnosis with T2DM at least 12 months prior to start the study
- Able to communicate; speak, read and write with Thai language fluently
- Able to provide written informed consent for participation
Exclusion Criteria
- Have been diagnosed with co-morbidities
- A lack of communication or cognitive dysfunction
- Require supports on their daily lives by other persons or care givers
- Transient migration from other communities
- Informed consent not provided.
