- •Patient must meet the so-called M2Q criterion (at least 2 quarters) coding for outpatient treatment must be done twice in different quarters, vaccination documentation available (patient management software or vaccination book), patients suffer from at least one of the following chronic somatic diseases: Asthma, Chronic obstructive pulmonary disease (COPD), breast cancer, coronary artery disease (CHD) or diabetes mellitus type 1 and 2.
