Brief Summary

The management of gestational diabetes mellitus focuses on dietary interventions, physical activity, and pharmacological therapy. Strategies that enhance adherence to the diet could play a pivotal role in managing this population. The use of digital tools has been gaining im-portance as an accessible tool to help control the disease. The objective of the study was to compare the effectiveness of outpatient nutritional guidance complemented by digital media with outpatient nutritional guidance in pregnant women with gestational diabetes mellitus. Randomized, patient-blind clinical trial. The pregnant women were randomly allocated into two groups: the Control Group (CG) received exclusively outpatient nutritional treatment and the Intervention Group (IG) received outpatient nutritional guidance and reminders via WhatsApp. Food intake (calories, carbohydrates, lipids, proteins and fiber) was assessed using 24-hour dietary recalls (24-hour R). Glycemic control and the need for pharmacological treatment were also assessed. 81 women were included, 34 allocated to the CG and 47 to the GI. Patients were followed for a period of 4 to 8 weeks. Few differences were observed in glycemic control between the two groups throughout the follow-up period. There were no statistical differences in pharmacological therapy (p=0.498). A statistically significant increase in lipid intake was observed in the IG throughout the follow-up period compared to the CG (p<0.001). There were no differences for insulin therapy, but glycemic control was better in the GI group. Furthermore, a increase in protein, fiber and lipid intake was noted in the intervention group.

Inclusion Criteria

Gestational diabetes mellitus according to the International Association Diabetes Pregnancy Study Group criteria; gestation between 24 and 34 weeks at the time of entry into the study; anticipated follow-up in the service for at least 4 weeks; single gestation with a live fetus; personal mobile phone; literate patient capable of using the WhatsApp application

Exclusion Criteria

Women who were unable to recall consumed food; pregnant individuals prescribed medication at the first appointment; patients with digestive system pathologies; a history of any type of bariatric surgery; or those who discontinued follow-up in the service in less than 4 weeks were excluded
