- age over 18 years old
- admit in ICU with acute respiratory failure require mechanical ventilator
- GCS > 10
- can receive enteral feeding
- Have delirium at the time of enrolment
- prior treatment of sleep disturbance and delirium or use continuous intravenous sedative for deep sedative effects or muscle paralysis drugs
- pregnant and breast feeding
- history of neurological disease, convulsion, psychiatric
- conditions likely to affect the enteral absorption such as intestinal obstruction, ileus, gastroparesis
- expect length of stag < 48 hr
- known allergic to melatonin
- re-admitted to ICU after randomized to study
