Brief Summary
The proportion of glycemic uncontrolled and controlled was 6:4 and the diabetic nephropathy was high that cause the renal failure. CCM is indicated concept for chronic care intervention

Inclusion Criteria
- •1) were diagnosed with type-2 DM, 2) in the age range of 30 to 80 years, 3) were attending at least one year and were supported by physicians from the diabetes care center, 4) were able to speak and read Thai.
Exclusion Criteria
- •1) patients who request withdrawals 2) unable participate such as inconvenient, become ill or had severe complications, move or die during research, and 3) incomplete information (clinical health record or questionnaire)
