Admission AAÜP and participation in the rehabilitative and transitional care programme (RÜP), German speaking,
existing informed consent

Exclusion Criteria

If a dcoumented denial to use the data exists, the affected patients will be excluded.

Primary Outcome Measures

Functioning, measured with the Barthel Index (Mahoney & Barthel 1995; Schädler et al. 2012) at admission into the acute and transitional care(T0), at discharge (T1) and six months after discharge (T3). <br>To measure funcionalty, the Barthel Index is a validated and common instrument. It includes 10 items that are rated on a 0, 5, 10 and 15 points Likert scale. A score of 100 points indicates maximum independence. <br>Data collection takes place with accurate monitoring the patients or interviewing patients, their families or nurses<br>

Independence measured with:<br>- Degree of dependency with the Resident Assessment Instrument, category Minimum Data Set Nursing Home Version at admission and discharge AAÜP (T0, T1) and 6 month after discharge AAÜP (T3) with the RAI Home Care Version (Anliker et al. 2009). <br>Data collection takes place with accurate monitoring the patients or interviewing patients, their families or nurses<br>- Amount of nursing services (Art. 7 <br> Krankenpflegeleistungsverordnung) ad <br> admission AAÜP (T0) and 6 months after <br> discharge (T3)<br>- Utilisation of the formal and informal Network<br>- Cognition (MMSE)<br>- Delir (DOS)<br>- Frailty (Frailty Index)<br>- Mobility (Short Physical Performance Battery)<br>- Fall efficacy (FES-I)<br>- Quality of Life
