• Completion of 24 weeks of participation in Study H9B-MC-BCDO or Study H9B-MC-BCDV, or have completed 100 weeks of participation in Study H9B-MC-BCDM
• Woman must not be pregnant, breastfeeding, or become pregnant during the study
