Periodontal diseasesare a group of infectious/inflammatory diseases of tooth supporting tissuesinitiated by putatively virulent microorganisms in the microbial dental plaque.1 Periodontaltissue destruction derives from a direct effect of the bacterial toxicsubstances and from the action of the immune system activated by the bacterialinfection.2 For an accurate diagnosis and a treatment choice, a reliable evaluationof periodontal inflammation is important.
The peptidylargininedeiminases (PADs) are a family of enzymes capable of converting amino acid arginineinto citrulline a process known as citrullination.4 Citrullinationis related to various physiological processes such as skin keratinization andgene regulation for immune functions, and also plays role in autoimmunediseases and inflammatory conditions.4.5Antibodies produced against citrullinated proteins are calledanti-citrullinated protein antibodies (ACPAs).5 It has been hypothesized thatperiodontitis may be a stimulator for ACPA formation.6 In addition, invitro studies have shown that one of the periodontal pathogens, Porphyromonas gingivalis,produces a variant of PAD (sometimes referred to as P. gingivalis PAD or PPAD),which is capable of citrullinating proteins.7 Studies have reportedthat increased expression of PAD2 in gingival connective tissue of patients withperiodontitis compared to periodontal healthy group.8 Therefore peptidylarginine deiminases can beused as a biomarker for the evaluation of periodontal disease.
This study is aimed atcomparing levels of PAD2 in gingival crevicular fluid of chronic periodontitispatients with before and after scaling and rootplaning alone and as an adjunctto scaling and root planing and flap surgery.

- Subjects with a minimum complement of 20 teeth.
- Subjects with chronic periodontitis indicated for flap surgery.
- Subjects with any systemic disease.
- Subjects who have undergone any periodontal treatment in the last 6 months.
- Subjects who have received antibiotics/anti-inflammatory drugs/steroids immunosuppressive chemotherapy within the last 6 months.
