CKD stages 3aâ€5 non-dialysis
Patients who had proteinuric kidney disease (microalbuminuria and proteinuria) with or without diabetes
Patients who was treated at rural hospital

Exclusion Criteria

patients who had kidney transplantation
Patient who undergone dialysis treatment
Patient who has Referred to provincial hospital
Pregnancy or breastfeeding
