Eligibility Criteria
Inclusion Criteria
- •The iCARE\-PD care model is open to all Parkinson's patients and their care partners, taking into account the exclusion criteria. The age of the patients and their caregivers does not matter, as does the gender or origin of the patients and their relatives.
Exclusion Criteria
- •Patients who meet one or more of the following criteria will be excluded from the study:
- •1\. Patients who are unable to give consent
- •2\. Patients who are in\-patient care in a nursing home
- •3\. Patients whose autonomy is restricted due to motor symptoms (Hoehn \& Yahr \= 5\)
- •4\. Patients who are restricted in their autonomy due to significant dementia diagnosed by a doctor
