CAR T-cell therapy involves collecting a patient's own T cells and genetically reprogramming them to target and eliminate disease-causing B cells. Unlike other treatments that only target B cells in the blood, CAR T-cell therapy also targets and eliminates those disease-causing cells in tissues, including within the nervous system. By removing these cells, the immune system may recover and reset, potentially leading to immediate and sustained control of autoimmune diseases.
Investigational use in progressive multiple sclerosis (MS). In March 2025, the first patient at Cleveland Clinic received CAR T-cell therapy for progressive MS in a Phase 1 clinical trial. A separate first-of-its-kind allogeneic CAR T-cell therapy trial for MS is underway at the University of Nebraska Medical Center (UNMC). Both trials remain in early phases, and researchers are still determining whether CAR T-cell therapy represents a viable treatment for MS and which patients might benefit most.
