Genetic Variants Linked to Treatment Response
The genome-wide association study (GWAS) uncovered several key genetic markers that influence GLP-1 (search) medication effects. Researchers identified a missense variant in the GLP1R (search) gene that is significantly associated with increased efficacy of GLP-1 medications for weight loss. This small change in the gene sequence alters the protein it produces, directly impacting how patients respond to treatment.
"I think we have proof of concept here that genetics is playing a role in terms of GLP-1 (search) efficacy and side effects," said Adam Auton, vice president at the 23andMe Research Institute (search) and senior author of the study.
Side Effect Prediction Through Genetic Analysis
The study also identified genetic associations with common GLP-1 (search) side effects. Variations in both the GLP1R (search) and GIPR (search) genes were linked to medication-related nausea (search) and vomiting (search), two of the most frequent adverse effects experienced by patients taking these drugs.
Notably, the research revealed drug-specific genetic effects. The association between GIPR (search) genetic variation and nausea (search) and vomiting (search) side effects was restricted specifically to individuals using tirzepatide (Mounjaro and Zepbound), but not semaglutide (Ozempic and Wegovy).
Clinical Variability and Predictive Modeling
The findings address a significant clinical challenge in obesity (search) treatment. While GLP-1 (search) receptor agonists, including semaglutide and tirzepatide, have transformed weight management, patients experience substantial variability in outcomes. Some individuals lose less than 5% of their body weight, while others achieve more than 20% weight reduction. Similarly, side effect experiences vary dramatically among patients.
The 23andMe research team successfully incorporated their genetic findings into a broader predictive model that includes demographic and clinical factors. This comprehensive approach demonstrates the ability to stratify patients by both weight loss efficacy and risk for certain side effects.
Among 23andMe research participants, the predictive model shows weight loss estimates varying between 6% and 20% of starting weight, with chances of nausea (search) or vomiting (search) ranging from 5% to 78%, depending on genetics and other factors.
Clinical Implementation and Patient Access
23andMe has released a new report called "GLP-1 (search) Medications Weight Loss and Nausea (search)" to members of its Total Health service. The report provides personalized insights into weight loss and nausea risk when taking GLP-1 medications based on the genetic findings. It includes an interactive tool allowing individuals to explore how genetics, age, and certain medical conditions may impact their treatment outcomes.
"While there is high interest in GLP-1 (search) medications, there is significant variation in how well they work for different people," said Dr. Noura Abul-Husn, Chief Medical Officer at the 23andMe Research Institute (search). "The market is crowded with weight loss support and medications, but the approach to weight management is typically one of trial and error. This can lead people to leap into treatment with a high degree of uncertainty and unrealistic expectations about efficacy and possible side effects."
The new GLP-1 (search) report is exclusively available through 23andMe's Total Health service, which includes clinician guidance. These reports are reviewed by clinicians and intended for use in a supervised clinical context, ensuring that genetic results are contextualized alongside patients' complete health profiles.
