Dr. Michael Roy, a professor of Medicine and deputy director of the Uniformed Services University's Military Traumatic Brain Injury (search) Initiative (MTBI2), presented findings on 3MDR — short for multi-modular memory desensitization and reconsolidation — a therapy designed to address the high dropout rates that plague standard PTSD treatments such as cognitive processing therapy and prolonged exposure.
How 3MDR Works
In a 3MDR session, the patient walks on a treadmill throughout the therapy while engaging with material they have personally selected: a song that pulls them back to the trauma, a calming song to conclude the session, and personal images that represent the traumatic memory. Each element is paired with a dual-task eye-movement exercise. The design gives patients a personal stake in their own treatment, directly targeting the adherence challenges that undermine conventional approaches.
Roy's team also prioritized affordability and portability. The therapy was adapted to run on a curved gaming monitor costing between $300 and $400, positioned in front of a standard treadmill. In the team's CARE for PTSD study, this configuration produced a 17-point drop on the PTSD Checklist.
MATE-3MDR: Adding Art and Music Therapists
A newer iteration, called MATE-3MDR, brings music and art therapists directly into the treatment process. These therapists help patients choose their music and create original artwork focused on post-traumatic growth. Among the first 10 patients to complete the program, PTSD Checklist scores fell by an average of 26 points.
Notably, every participant in the MATE-3MDR cohort carried both a PTSD diagnosis and a history of traumatic brain injury (search) — a pairing that is common among military personnel. The results suggest the approach holds up for service members managing both conditions simultaneously, a population for whom treatment options have been limited.
"For a service member who has walked away from talk therapy before, a version that reaches a clinic on a $300 screen could be the difference between starting over and finishing," Roy noted during the presentation.
The Broader Context
The 3MDR findings were part of a broader USU presence at MHSRS 2026, which drew more than 4,000 scientists, clinicians, and military leaders. The symposium's opening plenary featured Assistant Secretary of War for Health Affairs Keith Bass, who highlighted the Department of War's Warfighter Brain Health 2.0 initiative and the pressing need for tools that address the intersection of psychological trauma and brain injury in the force.
Roy also presented separate research at the conference — the BOATS study — examining blast overpressure and acceleration in crews of high-speed combatant craft, further underscoring USU's multi-pronged effort to understand and mitigate brain injury across the spectrum of military operations.
The MATE-3MDR results, while early, point toward a therapy model that is both clinically meaningful and logistically feasible, potentially offering a new pathway for service members who have not found success with traditional PTSD interventions.
