TOPAZ Clinical Trial Streamlines Breast Cancer Treatment, Offers Immediate Reconstruction

  • A young adult breast cancer survivor at MD Anderson experienced a shorter, easier treatment through the TOPAZ clinical trial.
  • The TOPAZ trial allowed for chemotherapy and radiation therapy followed by a double mastectomy with immediate reconstruction, avoiding painful spacers.
  • The patient has been cancer-free since February 2024 and experienced a smooth recovery with no lingering side effects.
  • The trial's success highlights the potential of clinical trials to improve treatment outcomes and quality of life for breast cancer patients.
A clinical trial at MD Anderson Cancer Center, known as TOPAZ, is offering a streamlined approach to breast cancer treatment, providing patients with the opportunity for immediate breast reconstruction and a reduced number of surgeries. This innovative approach is improving the treatment experience and outcomes for women facing this challenging diagnosis.

Streamlined Treatment with TOPAZ

The TOPAZ trial modifies the traditional treatment sequence for triple-negative breast cancer. Typically, patients undergo chemotherapy, followed by a mastectomy, then radiation therapy and breast reconstruction six months later, often involving painful spacers. The TOPAZ trial, however, integrates chemotherapy and radiation therapy, followed by a double mastectomy with immediate reconstruction. This eliminates the need for a second surgery and the intermediate step of using spacers.

Patient Experience

A 32-year-old patient diagnosed with stage III triple-negative breast cancer in May 2023 participated in the TOPAZ trial. She completed eight rounds of chemotherapy, followed by 20 rounds of radiation therapy. A double mastectomy with DIEP flap reconstruction was performed in February 2024. The patient has been cancer-free since February 2024 and has returned to her normal activities without lingering side effects.

Benefits of the TOPAZ Trial

The TOPAZ trial offers several potential benefits:
  • Reduced Number of Surgeries: Patients undergo one major surgery instead of two.
  • Immediate Reconstruction: Patients receive immediate breast reconstruction, avoiding the need for spacers and the emotional distress of being flat-chested.
  • Shorter Treatment Time: The integrated approach can potentially shorten the overall treatment duration.

Expert Perspectives

Dr. Clinton Yam, the patient's medical oncologist, explained that the goal of the TOPAZ trial was to make breast cancer treatment both shorter and easier. The patient expressed excitement about being part of something new and the potential for these clinical trials to open up new possibilities for people, whether it's making their breast cancer treatment easier or allowing them the option of immediate reconstruction.

Considerations for Clinical Trials

While clinical trials offer the potential for innovative treatments and improved outcomes, it's essential for patients to weigh the pros and cons and decide what outcome is most important to them. Consulting with medical professionals and understanding the trial's objectives and potential risks are crucial steps in making an informed decision.
