Abatacept Shows Long-Term Benefits in Delaying Rheumatoid Arthritis Onset by Up to Four Years

Researchers from King's College London have demonstrated that abatacept, an immunomodulatory drug already prescribed for rheumatoid arthritis (search) treatment, can delay disease onset by up to four years in high-risk individuals, according to findings published in Lancet Rheumatology.
The study represents an extended follow-up analysis tracking participants from an earlier 2024 study for four to eight years to assess the longer-term impact of abatacept intervention. The research team found that people at high risk of rheumatoid arthritis (search) who received abatacept took significantly longer to develop the disease compared with those given placebo.
Extended Benefits Beyond Treatment Period
The most significant finding was that benefits of abatacept lasted well beyond the one-year treatment period. Disease onset was delayed by up to four years beyond the treatment period, suggesting that early intervention can have lasting protective effects even after therapy discontinuation.
"Intervening early in people at high risk of RA can have lasting benefits," said Andrew Cope, professor of rheumatology at King's College London and lead author of the study. "We have shown that this approach is safe and can prevent disease while patients are on treatment as well as substantially relieve symptoms. Importantly, it can also delay the onset of RA for several years, even after treatment has stopped."
Mechanism and Clinical Impact
Abatacept works by targeting the cause of inflammation (search) in rheumatoid arthritis (search), an autoimmune condition where the body's immune system attacks healthy tissue, leading to joint inflammation that causes pain, swelling and stiffness. While the treatment did not permanently prevent rheumatoid arthritis, the study demonstrated it can significantly postpone disease development.
The clinical implications extend beyond symptom management. "This could reduce how long people live with symptoms and complications, drastically improving their quality of life," Cope noted.
Disease Burden and Diagnostic Challenges
Rheumatoid arthritis (search) affects approximately 400,000 people aged 16 and over in the UK. The condition presents with joint pain, swelling and stiffness as primary symptoms, but can also cause systemic effects including tiredness, lack of energy, high temperature, sweating, poor appetite, and weight loss.
Current diagnostic challenges compound the disease burden. "An accurate arthritis diagnosis often takes months or years, which is simply unacceptable," said Deborah Alsina, chief executive of Arthritis UK (search). "A prompt diagnosis followed by personalised treatment and support can make all the difference between someone being able to work and live an independent life, to losing their independence altogether."
Future Implications for Prevention
The research builds on a 2024 study that found abatacept was "effective in preventing the onset" of rheumatoid arthritis (search), but extends understanding of its long-term protective effects. The findings suggest a paradigm shift toward preventive intervention in high-risk populations.
"This research highlights how important it is to spot the early signs of arthritis to give us a chance at stopping it in its tracks, offering hope to thousands of people living with – or at risk of developing – rheumatoid arthritis (search)," Alsina added.
