All 50 States Join Medicaid Most Favored Nation Drug Pricing Model
核心洞察
The Trump administration announced that all 50 states, plus the District of Columbia and Puerto Rico, will join the Medicaid (搜索) most favored nation pricing model.
The White House projects nearly $65 billion in Medicaid (搜索) drug savings over the next decade from the arrangement, which ties selected drug net prices to lower prices paid abroad.
Twenty-six of the world's largest drugmakers, covering 90% of the branded U.S. market, have agreed to the pricing terms, according to the administration.
The Trump administration said all 50 states, along with the District of Columbia and Puerto Rico, will participate in a Medicaid (搜索) drug pricing model that applies most favored nation (MFN) pricing to selected medicines, bringing their net prices down to levels paid in certain other countries. The program is called the Medicaid "GENEROUS" model.
The White House projects the expansion will unlock nearly $65 billion in Medicaid (搜索) drug savings over the next decade. According to the administration, 26 of the world's largest drugmakers, covering 90% of the branded U.S. market, have agreed to bring U.S. prices in line with the lowest prices paid by comparable developed nations, with the agreements projected to save Americans more than $600 billion.
The announcement follows an executive order signed last year instituting a Most Favored Nations policy, under which the U.S. would commit to paying the same price for medications as the nation paying the lowest price anywhere in the world. The White House estimated in May that Trump's deals with pharmaceutical companies to lower some U.S. prescription drug prices to levels charged in other countries could save $529 billion over 10 years.
Medicaid mechanics and patient costs
Medicaid (搜索) enrollees already pay a nominal co-payment of a few dollars to fill prescriptions, so the direct effect on beneficiaries at the pharmacy counter may be limited. Lower net prices could instead ease pressure on state budgets that fund the state- and federally-funded program for people with low incomes.
The administration also points to the TrumpRx platform, which it says has delivered more than $700 million in savings and now covers more than 1,000 branded and generic medicines. Under that platform, GLP-1 (搜索) drugs that once cost over $1,000 a month start as low as $149, and eligible Medicare (搜索) seniors pay $50 a month, with more than 500,000 seniors already saving $216 million, according to the White House. Fertility drugs, inhalers, insulin and cholesterol treatments are among the high-cost categories the administration says have been cut by 50% to 90% or more.
Verification gap draws scrutiny
The scale of the claimed savings remains difficult to assess independently. Few details of the deals struck between the administration and participating pharmaceutical companies have been made public, and experts say the content of the agreements is not available for review.
"There is significant pressure on the administration to provide more granular data to justify its cost-savings claims," Kathy Hempstead, a senior policy advisor at the Robert Wood Johnson Foundation (搜索), told The Associated Press. She said the lack of information creates a barrier to legislative action.
"He is saying Congress should codify all of his MFN arrangements," Hempstead said. "But it's kind of unreasonable because it's not clear what Congress would actually do. No one knows what is in these deals."
Hempstead also raised the question of durability, noting concern about whether the current administration's tenure will affect the longevity of the price cuts and whether prices will revert once the administration leaves office.
Scope of the pricing relief
JD Hayworth, a former Republican congressman and spokesperson for the Pharmaceutical Reform Alliance (搜索), said in a statement that the announcement reflects "meaningful progress, but that progress remains incomplete," pointing to millions of Americans not enrolled in government healthcare who want lower drug prices.
Drug prices for U.S. patients depend on several factors, including the competition a treatment faces and insurance coverage. Most people have coverage through work, the individual insurance market, or government programs such as Medicaid (搜索) and Medicare (搜索), which shield them from much of the cost.
The administration framed the announcement as part of a broader effort to bring drug manufacturing back to the United States and secure the pharmaceutical supply chain.
