Arrowhead Pharmaceuticals Reports Strong Q1 2026 Results with REDEMPLO Launch Success and Promising Obesity Pipeline Data

Arrowhead Pharmaceuticals delivered strong financial results for the first quarter of fiscal 2026, reporting net income of $30.8 million compared to a net loss of $173.1 million in the prior year period. The biotechnology company's transformation was driven by $264 million in revenue, primarily from milestone payments and licensing agreements with strategic partners Sarepta Therapeutics and Novartis.
Commercial Launch Exceeds Expectations
The company's first commercial product, REDEMPLO (search) (plozasiran), achieved a robust launch following FDA approval in November 2025. Within just 10 weeks of market entry, the treatment for familial chylomicronemia syndrome (search) (FCS) generated over 100 prescriptions, surpassing management's initial expectations.
"We see this as a very strong start that exceeded our expectations for the early months of the launch," said Andy Davis, Chief Commercial Officer. The prescriptions demonstrated geographically balanced uptake across the United States, with preventive cardiologists and endocrinologists accounting for approximately 70% of total prescriptions.
Early patient demographics revealed that the overwhelming majority were APOC3-naive patients, with the remainder split evenly between those transitioning from expanded access programs and patients switching from competing therapies. Importantly, patients eligible for co-pay assistance paid $0 out of pocket during the fiscal first quarter.
REDEMPLO (search)'s clinical profile continues to differentiate the treatment in the market. In the Phase III PALISADE study, the therapy reduced triglycerides by 80% from baseline as early as month one, maintaining this reduction through 12 months of treatment. The FDA-approved prescribing information includes no contraindications, no warnings, and no precautions, with the therapy administered as a self-injection at home once every three months.
Breakthrough Obesity Data Shows Promise
Arrowhead reported encouraging interim results from its obesity (search) pipeline, particularly ARO-INHBE, which demonstrated significant weight loss enhancement when combined with tirzepatide in obese patients with type 2 diabetes (search). Two administrations of ARO-INHBE at the 400-milligram dose achieved approximately twofold better weight loss at week 16 compared to tirzepatide alone.
"This appears to be a high-quality weight loss as we saw an approximately threefold reduction in each of total fat, visceral fat and liver fat measures based on week 12 MRI versus tirzepatide alone in these patients," explained CEO Christopher Anzalone.
The company believes this represents the first demonstration in humans that the Activin E/ALK7 (search) pathway, a genetically validated pathway regulating adipose fat storage, may be therapeutically harnessed to improve body composition. This patient population typically experiences less weight loss with incretin therapy and requires more effective treatment options.
ARO-ALK7, the company's second obesity (search) candidate, showed dose-dependent reductions in adipose ALK7 (search) mRNA with a mean reduction of 88% at the 200-milligram dose at week 8, reaching a maximum reduction of 94%. Arrowhead believes this is the first RNAi therapeutic to demonstrate adipocyte gene target silencing in clinical trials.
Expanding Pipeline Across Multiple Therapeutic Areas
The company advanced its cardiometabolic portfolio beyond obesity (search) with ARO-DIMER-PA (search), a dual functional RNAi therapeutic designed to silence both PCSK9 and APOC3 genes simultaneously. This first-in-class approach targets mixed hyperlipidemia (search), a condition affecting approximately 20 million people in the United States who lack adequate treatment options.
"This represents an important step forward for the RNAi field as we believe it is the first clinical candidate to target 2 genes simultaneously in 1 molecule," Anzalone noted. The company expects interim data from this program in the second half of 2026.
In the central nervous system space, Arrowhead initiated dosing for ARO-MAPT (search), targeting tau protein (search) in the brain for potential treatment of Alzheimer's disease (search) and other tauopathies. The program utilizes the company's proprietary blood-brain barrier delivery system, enabling subcutaneous administration while achieving deep target gene knockdown across the CNS.
Strong Financial Position Supports Growth
The company significantly strengthened its balance sheet through multiple transactions totaling $1.33 billion in gross proceeds. This included a $200 million upfront payment from Novartis for ARO-SNCA, targeting alpha-synuclein (search) for Parkinson's disease (search) treatment, with potential milestone payments up to $2 billion.
Additionally, Arrowhead earned a $200 million milestone payment from Sarepta following successful drug safety committee review for ARO-DM1. The company also completed concurrent public offerings of $700 million in convertible senior notes and $230 million in common stock, both oversubscribed and priced on favorable terms.
Cash and investments totaled $917 million as of December 31, 2025, not including the additional $250 million expected from recent milestone achievements and anniversary payments. This financial strength positions the company to advance multiple programs toward commercialization while maintaining operational flexibility.
Regulatory Milestones on the Horizon
Looking ahead to 2026, Arrowhead anticipates several key value-creating events. The company expects Q3 2026 readout from Phase III SHASTA-3 and SHASTA-4 studies of plozasiran in severe hypertriglyceridemia (search), representing a potential $3-4 billion commercial opportunity affecting approximately 3.5 million patients with triglycerides above 500 mg/dL.
The FDA granted breakthrough therapy designation to plozasiran for severe hypertriglyceridemia (search), expediting development and review processes. The company remains on schedule to complete blinded portions of three Phase III studies by mid-2026, with planned supplemental NDA submission before year-end.
Total operating expenses increased to $223 million from $164 million in the prior year, primarily driven by Phase III registrational studies and commercial infrastructure investments. Nearly half of clinical trial spending supported the three registrational SHTG studies expected to read out this summer.
