Avacopan Demonstrates Superior Safety Profile in ANCA-Associated Vasculitis Across Multiple Clinical Trials

A post-hoc analysis of three randomized controlled trials involving 439 patients with granulomatosis with polyangiitis (搜索) (GPA) and microscopic polyangiitis (搜索) (MPA) showed that avacopan treatment resulted in substantially fewer adverse events compared to non-avacopan control regimens. The analysis, published in ACR Open Rheumatology, included 239 patients treated with avacopan and 200 patients who received control treatments across two phase 2 trials (CLEAR and ADVOCATE) and one phase 3 trial (CLASSIC).
"These results help confirm that, for patients with ANCA-associated vasculitis (搜索), avacopan is a well-tolerated medication with an excellent safety profile," said Peter A. Merkel, MD, MPH, chief of the division of rheumatology at the University of Pennsylvania.
The exposure-adjusted rates per 100 patient-years showed significant improvements with avacopan treatment. Adverse events were reduced by 151.9 (95% CI, 218.6 to 85.3), serious adverse events by 20.8 (95% CI, 38.3 to 3.3), white blood cell count reductions by 11.6 (95% CI, 22.2 to 1.2), and infections by 24.3 (95% CI, 48.5 to 0.1) compared to controls.
However, serious adverse events associated with hepatic function abnormalities were reported in 4.4% of avacopan-treated patients compared to 2.8% of controls, highlighting the need for liver function monitoring.
Real-World Safety Profile Analysis
A comprehensive pharmacovigilance study using the FDA Adverse Event Reporting System (FAERS) database analyzed 1,128 adverse event reports related to avacopan monotherapy from Q1-2022 to Q4-2024. The analysis revealed important patterns in adverse event occurrence and identified previously unreported safety signals.
The study found that female patients accounted for 47.0% of reports versus 28.4% for males, with the most frequent age group being 45-64 years (29.2%). The United States contributed the largest share of reports (89.3%), followed by Japan (5.1%) and Canada (2.7%).
Timing and Distribution of Adverse Events
The median time to onset of adverse events was 86.5 days, with 27.4% of adverse reactions occurring within the first month after medication initiation. Notably, approximately 10.4% of adverse reactions occurred after one year or longer of treatment, suggesting the need for both initial close monitoring and extended patient surveillance.
Severe outcomes were most commonly associated with hospitalization or prolonged hospital stay (19.5%), followed by death (6.9%).
Identified Safety Signals and Clinical Prioritization
Using two disproportionality analysis methods, researchers identified 33 adverse events associated with avacopan as valid signals. The most common adverse events aligned with the drug label and included nausea, fatigue, diarrhea, headache, rash, and upper abdominal pain.
The analysis revealed six System Organ Classes with valid signals, with gastrointestinal disorders ranking highest, followed by surgical and medical procedures, and infections and infestations.
Liver-Related Adverse Events
Drug-induced liver injury (搜索) emerged as a medium-priority safety signal, classified as a designated medical event. The study found that the incidence of liver dysfunction-related adverse events in East Asian populations was significantly higher than in Caucasian populations (p < 0.001).
Research indicates that risk factors for severe drug-induced liver injury (搜索) with elevated bilirubin in patients treated with avacopan include advanced age, lower body mass index, and early onset of liver injury after treatment initiation. One study suggested that gradually increasing the dose of avacopan in combination with ursodeoxycholic acid may help avoid liver injury risk.
Infection-Related Safety Concerns
Serious infections are included in the warnings and precautions section of the avacopan drug label. The pharmacovigilance analysis identified adverse event signals associated with infections, including localized infections and infectious pneumonia. The analysis identified five new potential adverse events not listed on the drug label: hair loss, increased appetite, muscle atrophy, esophageal disorder, and hyperesthesia teeth. Notably, alopecia was reported exclusively in patients from the Americas, with no Asian patients affected, suggesting a potential ethnic component.
Drug Interactions and Monitoring Considerations
Avacopan is both an inhibitor and substrate of CYP3A4 (搜索), requiring vigilance regarding drug interactions during treatment.
