For many patients, the journey involves years of misdiagnosis before receiving clarity.
One patient described a decade-long search for answers: "It took me around 10 years to finally get my indolent systemic mastocytosis (search) (ISM) diagnosis. My previous dermatologist had told me the reddish-brown spots on my chest and legs that would flare in the sun and heat were just age spots." It was only after moving and seeing a new doctor that the possibility of something more serious was raised.
The Standard Treatment Paradigm: Symptom Management
Treatment for ISM has historically focused on preventing and controlling specific symptoms and anaphylaxis. The cornerstone of therapy involves antihistamines that block histamine receptors. H1 antihistamines — such as cetirizine (Zyrtec), loratadine (Claritin), desloratadine (Clarinex), diphenhydramine (Benadryl), and hydroxyzine (Vistaril) — are used primarily for skin symptoms like itching and raised red streaks. H2 antihistamines, including famotidine (Pepcid), ranitidine (Zantac), and cimetidine (Tagamet), target histamine receptors predominantly found in the stomach to address gastrointestinal symptoms such as abdominal pain, nausea, diarrhea, and reflux.
When antihistamines prove insufficient, clinicians may turn to leukotriene antagonists like montelukast (Singulair) or zafirlukast (Accolate), oral cromolyn sodium (Gastrocom), or in refractory cases, short courses of oral corticosteroids such as prednisone. For bone complications — particularly osteoporosis, which can develop in ISM patients — bisphosphonates including alendronate (Fosamax), risedronate (Actonel), pamidronate (Aredia), and zoledronate (Reclast) are commonly prescribed, with denosumab (Prolia) serving as an alternative.
Crucially, all patients with ISM must carry self-injectable epinephrine for emergency treatment of anaphylaxis, a life-threatening complication that can occur following exposure to triggers or sometimes without any identifiable cause.
A Targeted Approach: Ayvakit (Avapritinib)
A paradigm shift in ISM treatment has arrived with Ayvakit (avapritinib), a medication that works by targeting the underlying genetic driver of the disease — the KIT D816V (search) gene mutation responsible for the overproduction of abnormal mast cells. Unlike conventional therapies that merely block the downstream effects of histamine release, avapritinib addresses the root cause.
One patient who learned about Ayvakit through an online support group described the impact: "In the support group, people were talking about a new drug just for mastocytosis called Ayvakit (avapritinib). I brought it to another new doctor I was seeing, an oncologist. He didn't know much about the drug but was supportive. It ended up being right for me, and we added it to my treatment plan, all because I was able to advocate for myself."
The patient reported that the medication "resolved or significantly improved my symptoms, including the body rash, itching, fatigue, brain fog, and joint and muscle pain." However, some limitations persist: "Even though the medication prevents a skin reaction to the sun, it doesn't completely eliminate the effects on my system as a whole."
For patients with severe symptoms that do not improve with standard treatments, other mast cell-reducing therapies may be considered, including interferon alpha-2b (Intron A) and cladribine (Mavenclad). Additionally, the injectable therapy omalizumab (Xolair) may help prevent anaphylactic episodes and reduce other ISM symptoms, though it is not FDA-approved specifically for ISM, which may affect insurance coverage.
The Critical Role of Self-Advocacy and Community
Given the rarity of ISM, many physicians lack familiarity with the condition, placing a significant burden on patients to educate themselves and advocate for appropriate care. Online communities, particularly through social media platforms, have become vital resources for information sharing and emotional support.
"Having a rare condition like ISM, I've had to do so much of my own research and find resources and support like the social media groups," one patient noted. "A lot of doctors I've seen aren't familiar with my condition and aren't willing to put in the time to learn or listen to what I have to say. It's easy to feel dismissed, which is why advocating for myself has become so important."
This self-advocacy extended to seeking care at specialized centers: "I asked my doctor in Fort Worth about possible drug trials I could join, which led me to MD Anderson (search). As a cancer and research center, this new environment of doctors is more curious and willing to figure things out. I feel much more supported here than with previous doctors."
Trigger Avoidance and Lifestyle Modification
Beyond pharmacotherapy, trigger avoidance remains a fundamental component of ISM management. Common triggers include heat, cold, temperature changes, exercise, sleep deprivation, stress and anxiety, alcohol, certain foods, specific medications such as aspirin and opiates, and bee or wasp stings. For one patient, this has meant significant lifestyle changes: "Before ISM, I loved being out in the sun — I could spend the whole day lying at the beach or working in my garden. Now, I have to be careful to stay in the shade and limit my time outside in the heat, which can be difficult in Texas."
The disease can also have cascading health effects. Long-term use of nonsteroidal anti-inflammatory drugs (NSAIDs) to treat ISM-related headaches, for instance, may contribute to kidney issues. Mental health impacts are similarly significant, with anxiety and stress recognized both as potential triggers and as symptoms of mastocytosis itself.
"We have to be our own number one advocate," the patient emphasized. "No one is going to do this for us; it's up to you to find out information, ask your questions, and battle the insurance companies. We have to dig a little deeper and fight for our health."
