Novel Mechanism Targets Endothelin Pathway
The acquisition centers on PER-001, an investigational small molecule endothelin receptor (搜索) antagonist currently in Phase II clinical development. The drug targets endothelin, described as "the most potent vasoconstrictor in the human body," which becomes upregulated in glaucoma (搜索), diabetic retinopathy (搜索), age-related macular degeneration, and retinal vein occlusion.
PER-001 is formulated as an intravitreal implant—a bio-erodible device administered into the vitreous cavity using a single-use, 25-gauge applicator. The sustained-release platform is designed to provide convenient dosing while delivering therapeutic levels of the drug directly to affected retinal tissues.
Addressing Significant Unmet Medical Need
Glaucoma (搜索) affects approximately 76-80 million people worldwide as of 2020, with projections indicating this number will reach 112 million by 2040 due to population aging. As the leading cause of irreversible vision loss, glaucoma represents a progressive optic neuropathy causing retinal ganglion cell loss and visual field deterioration. Despite its prevalence, no approved treatments currently exist that can prevent disease progression independently of lowering intraocular pressure.
Diabetic retinopathy (搜索) presents an equally compelling medical need, affecting about 146 million people globally—approximately 25 percent of adults with diabetes. This microvascular complication damages retinal capillaries, causing ischemia, leakage, and neovascularization. The condition is projected to affect 160 million people by 2045, with 25 million currently experiencing vision-threatening disease and 1.3 million people already blind from the condition.
Strategic Expansion of Ophthalmology Portfolio
The acquisition complements Bayer (搜索)'s existing ophthalmology expertise, particularly as the company faces patent expiration challenges with Eylea, its top-selling pharmaceutical product in 2025. Eylea now confronts pricing pressures following patent loss, making the Perfuse acquisition strategically important for pipeline reinforcement.

This acquisition represents Bayer (搜索)'s return to significant dealmaking after years of debt reduction following the $63 billion Monsanto acquisition in 2018. The company had largely avoided major transactions while focusing on cost reduction and operational streamlining, though it completed smaller acquisitions including Vividion Therapeutics for up to $2 billion and KaNDy Therapeutics for up to $875 million.
