Bayer Launches Phase III Trial Testing Mirena IUD for Precancerous Uterine Condition with No Approved Treatments

The SUNFLOWER study (Study of Understanding NAEH-treatment eFficacy of LevonOrgestrel–IUS in Women Electing non-suRgical options) will evaluate the efficacy and safety of Bayer's 52mg levonorgestrel-releasing intrauterine system (LNG-IUS) compared to oral progestin (search) therapy in treating NAEH.
Addressing a Critical Unmet Medical Need
Nonatypical endometrial hyperplasia (search) represents an abnormal thickening of the uterine lining caused by an imbalance between estrogen and progesterone levels. The condition is classified as nonatypical because the endometrial cells do not show signs of cancer, though it can progress to more serious forms if left untreated.
The annual incidence of NAEH ranges from 121 to 270 per 100,000 women, depending on geography, age, and menopause status. The condition can present asymptomatically or cause heavy or abnormal bleeding, including bleeding after menopause. Without treatment, NAEH may progress to atypical forms or even uterine cancer (search).

Study Design and Timeline
The SUNFLOWER trial aims to enroll 207 patients at approximately 90 centers across three countries. The study is anticipated to take around 1.5 years until completion, positioning it as a pivotal trial for establishing a new treatment paradigm for NAEH.
Previous studies testing the 52mg LNG-IUS for NAEH treatment have indicated good response rates, attributed to the type of hormone present in the device and its direct effect inside the uterus. These earlier investigations suggest the system helps restore the endometrium to a normal state in most cases.
Mirena's Mechanism and Current Applications
Mirena is a T-shaped device containing 52mg of synthetic levonorgestrel that continuously releases small amounts of the hormone directly into the uterus once placed. As a long-acting reversible contraceptive (LARC), it can be removed at any time by a healthcare provider if a woman's plans change.
The device's current approved indications, which may vary by country, include contraception, idiopathic menorrhagia/heavy menstrual bleeding (search), endometrial protection during estrogen therapy for menopause, and dysmenorrhea (search). Mirena is currently available in more than 120 countries worldwide.
